Overview

Epidural steroid injection for sciatica is a treatment used when irritation of the spinal nerves causes persistent leg pain. Nerves exit the lower spine and join to form the sciatic nerve which travels through the leg. When these nerves become inflamed, symptoms may include shooting pain, tingling, numbness, or weakness in the leg.

Common causes include disc bulges, spinal stenosis, or degenerative changes that place pressure on the nerve roots. Epidural steroid injection for sciatica delivers anti inflammatory medication directly into the epidural space surrounding the affected nerves. The aim is to reduce inflammation at the source, calm pain signals, and support recovery.

In some cases the injection can also help confirm whether nerve inflammation is the main cause of symptoms.

Who this treatment helps

Epidural steroid injection for sciatica may be considered for people with:

  • Sciatica or leg pain caused by nerve irritation
  • Disc bulges or herniated discs affecting spinal nerves
  • Spinal stenosis causing nerve compression
  • Persistent leg pain that has not improved with conservative care
  • Pain limiting walking, sleep, or rehabilitation

A consultant assessment is required to confirm whether this treatment is appropriate for your condition.

What is an epidural steroid injection

An epidural steroid injection involves guiding a fine needle into the epidural space of the spine using imaging guidance. A mixture of steroid medication and local anaesthetic is then injected around the irritated nerves.

The steroid helps reduce inflammation while the local anaesthetic may provide temporary relief and help confirm the pain source. Although the procedure does not correct structural spinal changes, reducing nerve inflammation may significantly ease symptoms and allow improved movement.

What to expect during the procedure

Your consultant will explain the procedure and answer any questions before treatment begins.

  1. Consultant review and confirmation of the treatment plan
  2. Comfortable positioning on the procedure table
  3. Local anaesthetic used to numb the skin
  4. Image guided needle placement into the epidural space
  5. Injection of steroid and local anaesthetic medication
  6. Short monitoring period before discharge

The procedure is usually completed within a short period and is commonly performed as a day case treatment.

After the procedure

After an epidural steroid injection for sciatica, temporary numbness or heaviness in the leg may occur and usually settles within a few hours.

  • Mild soreness at the injection site is common
  • Pain relief may begin within several days
  • Gradual improvement in symptoms may occur over the following weeks
  • Physiotherapy or rehabilitation is often recommended

Your consultant may also advise gradual return to normal activity while avoiding strenuous exercise for a short period.

Benefits

  • Reduces inflammation around irritated spinal nerves
  • May ease leg pain and sciatica symptoms
  • Minimally invasive procedure
  • Short recovery period
  • Can support rehabilitation and improved mobility

Possible risks and side effects

Epidural steroid injections are generally safe when performed by experienced clinicians, but all procedures carry some risk.

  • Temporary increase in pain
  • Local soreness or bruising
  • Temporary numbness or weakness
  • Infection which is rare
  • Bleeding which is uncommon
  • No meaningful improvement in some cases

Your consultant will explain the potential risks and expected benefits before the procedure.

When to consider other options

If symptoms do not improve or if a different cause of nerve irritation is suspected, other treatments may be discussed. These may include lumbar nerve root block and PRP, epidural lysis of adhesions and PRP, rehabilitation programmes, medication review, or further specialist assessment.

Safety note

Please inform your consultant about medications, allergies, diabetes, bleeding disorders, active infection, or pregnancy before the procedure. Depending on the medications used, you may require transport home after treatment.
