Overview

Piriformis muscle injection may be considered when buttock and leg pain appears to arise from the piriformis muscle rather than the spine. Piriformis related leg pain often worsens with sitting, climbing stairs, or prolonged activity, and symptoms can resemble sciatica. In some cases, spinal imaging is normal or does not fully explain the pattern of pain.

Piriformis muscle injection aims to relax the piriformis muscle, reduce spasm, and relieve pressure on the sciatic nerve. This may improve movement, reduce pain, and support more effective rehabilitation. In selected cases, Botox may be considered when longer lasting muscle relaxation is needed.

What is a piriformis muscle injection or Botox

A piriformis muscle injection is a targeted procedure in which medication is delivered directly into the piriformis muscle using imaging guidance. The injection may include local anaesthetic, steroid medication, or both, depending on the clinical picture.

Botox may be used in selected cases to provide longer lasting muscle relaxation. The aim is to reduce muscle spasm, ease pressure on the sciatic nerve, and improve sitting, walking, and general movement tolerance.

What to expect during the procedure

Your consultant will explain the treatment plan and confirm the target area before the procedure begins.

  2. Comfortable positioning for the procedure
  3. Skin preparation and local anaesthetic where appropriate
  4. Image guided injection into the piriformis muscle
  5. Short observation period before discharge

After the procedure

It is normal to experience some temporary soreness after piriformis muscle injection.

These may include epidural steroid injection, lumbar nerve root block and PRP, rehabilitation, medication review, or further investigation depending on the cause of symptoms.
